Abstract
Purpose
This study aimed to evaluate and compare the accuracy of the size of the posterior malleolar fragment measured using lateral plain radiography and three-dimensional computed tomography (3DCT) in patients with ankle trimalleolar fractures.
Materials and Methods
This study enrolled 80 patients (80 ankles) with ankle trimalleolar fractures and analyzed the size of the posterior malleolar fragments using plain radiography and 3D-CT. The articular involvement of the posterior malleolar fragments was measured as a percentage of the articular surface in the sagittal length of the tibial plafond using lateral plain radiography, and the articular surface area was directly measured using 3D-CT. In addition, we classified the patients into three groups based on the morphology of the posterior malleolar fracture, according to the Haraguchi classification method, and evaluated and compared the accuracy of the size of the posterior malleolar fragments.
Results
The mean articular involvement of the posterior malleolar fragments on plain radiography was 27.6% (range, 6.0%-53.1%), which was significantly higher than the mean of 21.9% (range, 4.7%-47.1%) measured using 3D-CT (p=0.004). In the analysis, according to the fracture morphology, the mean difference between the two methods was the largest for type I fractures at 9.1% (range, 1.8%-19.5%) and the smallest for type II fractures at 1.1% (range, –7.7% to 8.8%).
Conclusion
The articular involvement of posterior malleolar fragments measured using plain radiography showed low accuracy and significantly higher values than the actual articular involvement. Therefore, careful evaluation using 3D-CT is crucial for accurate analysis and optimal treatment in patients with ankle trimalleolar fractures.

목적
족관절 삼과 골절에서 측면 단순 방사선 사진과 3차원 컴퓨터 단층촬영을 이용한 후과 골편의 크기 측정의 정확도를 비교하고자 시행되었다.
대상 및 방법
총 80명(80예)의 족관절 삼과 골절 환자를 분석하였으며, 후과 골편의 크기 측정은 측면 단순 방사선 사진과 3D-CT를 이용하여 시행되었다. 후과 골편의 관절면 침범 정도는 측면 단순 방사선 사진에서는 경골 천장부의 시상면 길이에 대한 비율로 측정하였고, 3D-CT 영상에서는 경골 천장부 면적에 대한 후과 골편 면적의 비율로 측정되었다. 그리고 후과 골절을 Haraguchi 분류법에 의해 세 군으로 분류하고 각 군에 따른 두 측정법의 정확도 차이를 비교하였다.
결과
측면 단순 방사선 사진을 통한 후과 골편의 평균 관절면 침범 정도는 전체 경골 천장 면적의 27.6% (범위 6.0%-53.1%)로 3D-CT를 이용하여 측정한 평균 크기인 21.9% (범위 4.7%-47.1%)보다 유의하게 크게 측정되었다(p=0.004). 후과 골절 형태에 따른 분석에서 두 측정법의 평균 차이는 type I 골절에서 평균 9.1%로 가장 컸고, type II 골절에서 평균 1.1%로 가장 작았다.
결론
측면 단순 방사선 사진을 통한 후과 골편의 평균 관절면 침범 정도는 실제 관절면 침범 정도에 비해 유의하게 큰 측정값을 보여 낮은 정확도를 보여주었다. 따라서 족관절 삼과 골절에서 후과 골편의 크기를 정확하게 측정하기 위해서는 3D-CT를 이용하여야 한다.

족관절 골절 중 후과 골절을 동반한 경우는 많게는 44%에 이르는 것으로 보고되며, 주로 족관절 주위 회전 손상에 의해 발생한다.1,2) 생역학적 연구에 따르면 후과는 여러 인대의 부착부이자 원위 경골 관절면을 이루는 구조물로써, 체중 부하 전달, 족관절의 회전 및 거골의 후방 전위에 대한 안정성에 중요한 역할을 한다.3,4) 이에 족관절 골절 중 후과 골절을 동반한 경우는 그렇지 않은 경우에 비하여 임상적 결과가 불량하고 향후 족관절 퇴행성 변화의 가능성이 높은 것으로 알려져 있다.2,5,6,7,8)
후과 골절에 대한 적절한 치료법에 대해서는 아직 논란이 있으나 골편의 크기, 분쇄 정도 및 관절면 침범 정도, 비골 구(fibular incisura)의 침범 및 원위 경비인대결합의 안정성 등이 중요한 역할을 한다.4,8,9,10,11) 이 중에서도 골편의 크기 및 관절면 침범 정도는 수술적 치료 여부를 정하는 중요한 요소이며, 일반적으로 골편이 원위 경골 관절면의 25%-30% 이상을 침범하는 경우 내고정술의 대상이 된다고 알려져 있다.2,5,6,12,13)
현재까지는 후과 골편의 관절면 침범 정도에 대한 평가는 주로 족관절 측면 단순 방사선 영상을 이용하여 경골 원위부의 족관절 관절면에 대한 후과 골편의 관절면 침범 비율(%)을 측정하여 평가하였다. 하지만 단일 평면 기반의 측정법은 다양한 후과 골절의 형태 및 부정확한 측면 단순 방사선 영상 등으로 인해 영향을 받을 수 있으며, 최근 일부 연구에서 측면 단순 방사선 영상을 이용한 후과 골편 크기의 평가에 대한 정확도는 낮은 것으로 보고하였다.11,14,15,16)
본 연구는 측면 단순 방사선 영상을 이용한 후과 골편 크기의 측정은 후과 골절의 형태에 따라 정확도가 떨어질 수 있다는 가설하에 분석을 진행하였다. 이에 족관절 삼과 골절 환자에서 측면 단순 방사선 영상과 3차원 컴퓨터 단층촬영(three-dimensional computed tomography, 3D-CT) 영상을 이용하여 측정한 후과 골편 크기의 정확도를 평가하고 비교하고자 한다.
1. 연구 대상 및 방법
2015년 1월부터 2019년 12월 사이에 전남대학교병원에서 회전 손상력에 따른 Lauge-Hansen 분류 회외-외회전(supination-external rotation, SER) IV 단계 또는 회내-외회전(pronation-external rotation, PER) IV 단계 손상의 삼과 골절 환자 중 수술 전 족관절 3D-CT를 시행하고 수술치료를 받은 환자를 대상으로 하였다. 단, 3D-CT를 통해 후과 골편의 원위 경골 관절면 침범 정도를 직접 측정할 수 없거나 관절면의 분쇄가 심한 경우 및 측면 단순 방사선 영상에서 원위 비골과 경골 후방이 겹치지 않거나 거골 상부 관절 표면이 보이는 경우는 부적절하게 영상이 측정된 경우로 판단하여 연구에서 제외하였다. 최종적으로 총 80명(80예)의 환자를 선정하였으며, 평균 나이는 49.8세(범위 18-83세)였다(Table 1). 손상 기전에 따른 분류로는 회외-외회전 골절(SER)이 50예(62.5%)이며 회내-외회전 골절(PER)이 30예(37.5%)였다.
후과 골편 크기에 대한 측정은 원위 경골 관절면 침범 정도를 평가하였으며 대상 환자의 수술 전 족관절 측면 단순 방사선 영상과 3D-CT 영상을 이용하여 측정하였다(Fig. 1). 측면 단순 방사선 영상을 이용한 측정법은 원위 경골 관절면에서 후과 골절선을 기준으로 전후 관절면 길이를 측정 후 후과 골편이 차지하는 비율로 계산하였으며, 3D-CT를 이용한 측정법은 원위 경골 관절면을 확인할 수 있는 재건 영상에서 PACS (picture archiving and communication system) (Maroview 5.4; INFINITT Healthcare, Seoul, Korea)의 ROI (region of interest) tool을 사용하여 전체 원위 경골 관절면에서 후과 골편의 면적이 차지하는 비율을 계산하였다.
더불어 CT 축상면 영상에 따른 후과 골절의 형태에 따라 Haraguchi 분류법에 의해 다음과 같이 type I (후외과-사선형), type II (내과-확장형), type III (후외피형)의 3가지 형태로 분류하였으며 이에 따른 두 측정법 간의 결과를 비교하였다(Fig. 2).11)
모든 영상학적 측정값은 세 명의 정형외과 의사에 의해 측정되었으며, 세 명의 관찰자 간의 측정치에 대한 신뢰도(reliability)를 분석하였다. 본 연구는 본원 임상연구윤리위원회(Institutional Review Board)의 승인을 받았으며(CNUH-2021-216), 서면동의서는 면제받았다.
2. 통계 분석
통계 분석에는 Windows용 IBM SPSS Statistics 23.0 프로그램(IBM, Armonk, NY, USA)을 사용하였다. 후과 골편의 크기에 대한 분석에서는 세 명의 관찰자 간 측정치의 평균값을 적용하여 분석하였으며, 세 명의 관찰자 간의 측정치에 대한 일치도를 분석하였다. 두 군 간의 연속 변수에 대한 분석으로는 Kolmogorov–Smirnov 검정을 통한 정규성 확인 후 독립표본 t-검정을 이용하였으며, 이분법적 변수에 대한 분석에서는 χ2 검정을 이용하였다. Pearson 상관분석법을 이용해 측면 단순 방사선 영상을 통해 측정한 후과 관절면 길이와 3D-CT를 통해 측정한 후과 골편의 관절면 면적 간의 연관성을 분석하였다. 모든 통계 분석은 통계학자에 의해 검토되었으며, p값이 0.05 미만일 경우 유의성이 있는 것으로 하였다. 추가적으로 G*Power 3.1.9.2를 이용하여 post hoc power analysis를 시행하였다.
1. 후과 골절의 형태 및 관절면 침범
총 80예의 삼과 골절에서 수술 전 촬영한 3D-CT 영상에 기반한 후과 골절의 Haraguchi 분류법상 type I은 42예(52.5%), type II는 27예(33.8%), type III는 11예(13.8%)를 차지하였다(Table 1).11)
측정방법에 따른 관절면 침범 정도는 측면 단순 방사선 영상을 이용한 경우 평균 27.6% (범위 6.0%-53.1%)로 3D-CT 영상을 통해 측정한 평균 21.9% (범위 4.7%-47.1%)보다 평균 5.7% (범위 –7.7%-19.5%) 크게 측정되었으며, 두 측정방법 간에 통계적으로 유의한 차이를 보였다(p=0.004) (Table 2). 두 측정방법에 의한 평균 관절면 침범 정도에 기반한 post hoc power analysis에서 검정력은 87.6%로 조사되었다.

후과 골절 형태에 따른 관절면 침범 정도에 대한 분석에서는 측면 단순 방사선 영상을 이용한 경우 type I은 평균 9.1% (범위 1.8%-19.5%), type III는 평균 4.4% (범위 0.1%-8.4%)로 유의하게 큰 측정값을 보고하였으나, type II의 경우 두 측정법 간 차이는 평균 1.1% (범위 –7.7%-8.8%)로 유의한 차이는 보이지 않았다. 후과 골편의 길이와 면적 간에는 type I, II, III 모두에서 통계적으로 유의한 양의 상관관계를 보였다(Fig. 3).
2. 관찰자 간 신뢰도
세 명의 관찰자가 측정한 관절면 침범 정도에 대한 두 측정법의 급내 상관계수(intraclass correlation coefficient, ICC)는 모두 0.8 이상으로 만족스러운 신뢰도(reliability)를 보고하였다(Table 3). 하지만 후과 골절의 형태에 따른 분석에서는 비교적 골편의 크기가 작은 type III의 경우 단순 방사선을 이용한 측정법의 ICC는 0.84로 다소 낮았다.

족관절 삼과 골절에서 후과 골편의 형태 및 관절면 침범 정도는 수술적 치료의 방향을 결정하는 중요한 요소 중 하나이며, 이를 평가함에 있어 높은 정확도와 신뢰도가 요구된다. 본 연구 결과에 따르면 단순 방사선 영상에 기반한 측정법은 후과 골편의 크기를 과대평가하는 것으로 조사되었다.
일반적으로 원위 경골 관절면의 25%-30% 이상을 침범하는 후과 골편의 경우 내고정술의 대상이 된다고 알려져 있으며, 이에 후과 골편의 관절면 침범 정도에 대한 정확한 평가는 매우 중요한 부분이다.2,5,6,12,13) 하지만 최근 기존의 단순 방사선 영상을 이용한 후과 골편의 관절면 침범에 대한 측정법의 정확도에 일부 의문이 제기되고 있다.11,14,15,16) Büchler 등16)은 단순 방사선 영상에 기반한 후과 골편의 크기 측정법은 정확도 및 신뢰도가 낮으며, 특히 골절부의 분쇄 및 감입을 동반할 경우 과소평가할 수 있다고 보고하였다. Meijer 등14)도 31예의 족관절 삼과 골절을 대상으로 단순 방사선 영상과 3D-CT 영상의 비교분석을 통해, 단순 방사선 영상을 이용한 측정법의 경우 평가자 중 84%에서 후과 골편의 크기를 과대평가하였으며, 이는 3D-CT를 이용한 측정값의 약 1.8배로 두 측정법 간 유의한 차이를 보고하였다. Black 등17)도 100예의 족관절 골절에 대한 분석을 통해 후과 골절을 평가함에 있어 단순 방사선 영상의 낮은 정확도로 인해 추가적인 CT 영상 검토 후 24%에서 수술적 치료법의 변경이 있었다고 보고하였다.
단순 방사선 영상을 통한 측정법의 낮은 정확도와 신뢰도는 다양한 원인에 기인한 것으로 판단된다. 먼저, 후과 골절은 단순히 후외측에 국한되지 않으며 다양한 골절 형태를 갖는 경우가 많다. 이러한 다양한 골절 형태로 인해 골편의 크기를 2차원적 단순 방사선 영상만으로 정확히 파악하기에는 무리가 있으며, 골절부의 분쇄 및 골편의 감입 등이 동반될 경우 이를 더욱 어렵게 한다. 또한, 평면이 아닌 곡선 형태의 원위 경골 관절면을 직선으로 단순화하여 측정한다는 점도 정확한 관절면 침범 정도 측정에 있어 한계점이다. 마지막으로 단순 방사선 영상의 경우 환자의 자세와 방사선 주사선(beam) 방향과 골절선과의 관계에 따라 골편의 크기가 과대 또는 과소평가될 수 있는 가능성이 있다.14,18) 본 연구에서도 이와 같은 결과를 확인할 수 있으며, 골절선의 방향이 방사선 주사선과 평행하지 않고 사선형을 갖는 type I 골절의 경우 두 측정법 간의 차이는 최대 19.5%까지 보고되었으나 골절선이 비교적 일정하게 내측까지 연장된 type II 골절의 경우 두 측정법 간 유의한 차이를 보이지 않았다. 이는 2차원적인 단순 방사선 영상의 특성상 type I 골절의 경우 후외과 골절부의 음영이 투사되어 내측까지 연장된 것으로 판단되기 때문에 골절 부위를 과대평가할 수 있게 되기 때문이다. 반면 type II 골절의 경우 골절선이 비교적 반듯하게 내측까지 연장되며 골절선의 방향이 방사선 주사선과 거의 평행하기에 측면 단순 방사선 영상을 통한 측정법도 3D-CT를 통한 측정값 간에 유의한 차이를 보이지 않은 것으로 판단된다.
이 외에도 단순 방사선 영상에 기반한 측정법의 경우 3D-CT를 이용한 측정법에 비하여 상대적으로 낮은 관찰자 간 일치도를 보인다고 알려져 있다.14,16) Haraguchi 등11)은 후과 골절을 3가지 형태로 분류한 후 이에 따른 관찰자 간 일치도를 분석한 결과 0.61의 낮은 ICC를 보고하였으며, Büchler 등16)도 골절의 형태 및 분쇄에 따라 다양한 일치도를 보인다고 하였다. Meijer 등14)도 단순 방사선 영상에 기반한 후과 골절의 진단에 대한 관찰자 간 일치도(ICC)는 불량한 것으로 보고하였고, 이러한 문제점 등으로 인해 술자는 후과 골절의 형태 및 크기를 판단함에 있어 더 이상 단순 방사선 영상에만 의존해서는 안 되며, 반드시 수술 전 3D-CT를 통한 골절의 정확한 분석이 필요하다고 주장하였다. 본 연구에서는 두 측정법 모두 0.8 이상의 비교적 만족스러운 일치도를 보였다.
본 연구의 한계점으로는 비교적 증례가 적고, 후향적 연구로 인해 동일한 조건하에 영상학적 검사를 시행하지 못했다는 점이다. 둘째로, 일부 단순 방사선 측면 영상이 부적절하게 측정된 경우, 후과 골편의 분쇄 또는 감입 골편을 동반하는 경우를 연구 대상에서 제외함에 따른 선택 편향(selection bias)이 작용했을 수 있다. 마지막으로 관찰자 내 신뢰도에 대한 추가적인 분석을 하지 않았다는 제한점이 있다.
단순 방사선 영상에 기반한 후과 골편의 관절면 침범 정도에 대한 측정법은 3D-CT 영상을 통한 측정법에 비하여 낮은 정확도를 보이며 유의하게 큰 측정값을 보고하였다. 따라서 후과 골절이 동반된 족관절 골절에서 정확한 분석 및 적절한 치료 계획 수립을 위해서는 3D-CT를 통한 세심한 평가가 필요하겠다.
