Operative Treatment of Bilateral Tibial Tuberosity Fractures in Adolescent: A Case Report

Bilateral avulsion fractures of the tibial tubercles are extremely rare. There is no case report about this in Korean literature. We present simultaneous bilateral tibial tuberosity fractures in 14-year-old adolescent male fell on the ground during running. These fractures were managed by open reduction and screw fixation. We gained complete union and removed metal after 6 months. Functional results were excellent 6 month after surgical treatment.

Galeazzi fractures in child is rare and seldom necessary of operative treatment because the result of conservative treatment is good. We present the patient who was a 11-year-old male and fell onto his both hands during a hundred-meter dash. His diagnosis was bilateral Galeazzi fractures and limited open reduction and internal fixation with Kirschner pins was initial treatment at local hospital. After 4 weeks postoperatively, Kirschner pins were removed and rehabilitating exercise was started. After 4 months postoperatively, he was transferred to our hospital due to malunion with severe angular deformities and distal radioulnar joint (DRUJ) dislocation. He was treated with corrective osteotomy. Thus, as in this case, we suggest more careful treatment and observation if conservative method of Galeazzi fracture in child is chosen and consider operative method as treatment according to age and pattern of fracture.

Posterior cruciate ligament avulsion fracture is occurred by high energy trauma, usually in motor vehicle accident or sports injury. Bilateral posterior cruciate ligament avulsion fracture is not yet reported in Korea. Authors report a case of bilateral posterior cruciate ligament avulsion fracture in 16-years-old man treated with anatomical reduction and internal fixation with a review of literature.

Traumatic anterior dislocation of the hip is an uncommon injury, accounting for less than 10% of all reported cases of traumatic hip dislocation. Especially, there are no known report in our country so far. We are reporting a case of a 81 year old man who sustained bilateral anterior hip dislocation after pedestrian traffic accident, and treated by closed reduction and skeletal traction at our institute.

Traumatic simultaneous bilateral hip dislocation is reported rarely, but the most of them are limited in young patients. The authors managed the elderly patients whose both hip was dislocated traumatically, simultaneously and who didn't have any other underlying disease and other associated fracture - femur, hip joint and pelvis, with a review of the relevant literature.

Clinical Distribution of Bilateral Non-contemporary Hip Fractures in Elderly Patients

PURPOSE
To evaluate the clinical distribution of bilateral non-contemporary hip fractures regarding to fractures type, risk factors and fractures interval in elderly patients.
MATERIALS AND METHODS
24 bilateral non-contemporary cases among 621 hip fractures from Sep. 1997 to Dec. 2004 were evaluated regarding to gender, age, incidence, Singh index, causes, interval, fracture pattern, operative methods and underlying diseases between the two fractures retrospectively.
RESULTS
The distribution is as follows: males to females (5:19), incidence (3.86%), mean age (76.9 years and 78.9 years), average Singh index 2.5 degree (2~4) and 2.1 degree (1~3) respectively. 21 cases (87.5%) in both fractures suffered from minor slips and 19 cases (79.1%) occured within 3 years of the first fracture and 17 (68.1%) cases were same type fractures. Bipolar hemiarthroplasty was performed in 12 cases. All but one patient had underlying cardiovascular diseases and CVA sequales.
CONCLUSION
To prevent the bilateral non-contemporary hip fractures, surgeons must bear in mind that osteoporosis treatment, control of underlying cardiovascular diseases and CVA sequales, and the effective rehabilitation is very important.

Morbidity and Mortality of Bilateral Hip Fractures in Elderly Patients

PURPOSE
The purpose of this study was to estimate the morbidity and mortality rate of bilateral hip fractures in elderly patients compared to that in unilateral hip fractures and to evaluate it's related risk factors.
MATERIALS AND METHODS
Twenty-two cases of bilateral hip fractures in patients who were older than 70 years with at least two year follow-up were included in our study. We analysed the risk factors of bilateral hip fractures by comparing with age, sex and diagnosis matched 22 cases of ipsilateral hip fractures including onset of secondary fracture, injury mechanism and the rate of morbidity and mortality, respectively.
RESULTS
The onset of secondary fracture and death were mostly within 1 year after operation for the first hip fracture. Comorbidity of cardiovascular, neurologic, urologic or history of previous fracture and decreased ambulation ability were related with the occurrence of bilateral hip fractures. The rate of morbidity and mortality of bilateral hip fractures were about two- fold than that of ipsilateral hip fractures. High mortality rate was noted in patients who had operation delay from injury. But no significant relationship between nutrition, body weight or bone mineral density and the development of secondary hip fractures.
CONCLUSION
To prevent the occurence of bilateral hip fractures which had more serious results than that of ipsilateral hip fractures, more aggressive rehabilitation to improve walking ability and appropriate environmental circumstances to avoid falls were important, especially in older patients.

A rare case of traumatic bilateral anterior and posterior dislocations of the hips occurred by passenger traffic accident . The right hip was dislocated posterosuperior to the right acetabulum with a linear acetabular posterior column tracture(Thompson and Epstein type IV) and left hip was dislocated anteroinferior to the left acetabulum(modified classification of Epstein type II A). The dislocations were successfully reduced by the Bigelows method for the right hip and the reverse Bigelows method for the left hip, and 4 weeks of Bucks traction was applied. He was able to return to full activity after 4 months. There was no sign of avascular necrosis at 3 years and 11 months follow-up.

Hip dislocation represents 2 to 5% of all joint dislocations. Bilaterat dislocation of the hip joints is reported about 1.25% of all cases fo hip dislocations and therefore 0.025 to 0.050% of all joint dislocations. Dislocations in which one hip dislocates anteriorly and the other posteriorly are even rarer. Of all traumatic bilateral hip dislocations, bilateral simultaneous anterior and posterior dislocations in 40% of cases. This paper is a case report of a traumatic bilateral anterior and posterior dislocation of hips in a 24 year-old man injured by motor vehicle accident as a passenger. The patient was treated by means of closed reduction, traction and physical therapy. We report such a case.

Bilateral Femoral Fractures in Adults

Recently, the incidence of multiple trauma combined with cerebral injury and multiple long bone fractures has been increased associatied with the high rate of automobile and the industrial accident. When the femur was fractured, it is difficult to reduce accurately and immoblize due to the displacement, because this bone is responsible for the weight bearing and surrounded by the strong muscle groups. Especially, bilateral fracture of femur which was usually produced by high energy trauma was combined with the other injuries. So the treatment & rehabilitation were difficult and recovery period was prolonged. 11 cases with both femoral fractures which were treated at Department of Orthopaedic Surgery, Ewha Womans University Hospital from January, 1984 to December, 1988 were clinically analyzed and the results were as follows. 1. The prevalent age ranged from 20 to 29 years. 2. Among 11 cases, 7 cases were male nad 4 cases were female. 3. The cause of injury was mainly the dash board injury due to the traffic accident. Of 11 cases, 10 were combined with injuried of another sites. 4. The union time was varied from 12 weeks to 18 weeks and average time was 15 weeks. 5. The complication were limitation of motion of knee joint in 5 cases, nonunion in 2 cases, delayed union, angulation and metalic migration in case, each other. 6. Protected parial wt. bearing was performed between 12 weeks and 20 weeks and non protected full wt. bearing was performed between 6 months and 26 months after operation.
