The Prognosis of Undisplaced Abduction Fracture of the Neck of the Femur

PURPOSE
The aim of this study was to analyze frequencies, types, and treatment results of the undisplaced abduction fracture of the neck of the femur for the prediction of the prognosis. MATERIALS & METHODS: From February 1984 to February 1999, the population was selected from those who were admitted in our hospital with the diagnosis of the fracture of the neck of the femur(96 cases). Among them 34 cases of undisplaced abduction fracture who could be followed minimum 2 years were chosen. The types of initial injury, the degrees of the rotation into valgus, union and the incidences of avascular necrosis of the femur head were analyzed by plain radiographs and medical records.
RESULTS
The undisplaced abduction fracture of the neck of the femur were 34 cases(35%) of the whole 96 cases and 33 cases of them had been operated. In all the 34 cases we were able to see the bone union, however in 7 cases(20.6%) the avascular necrosis of the femur head were happened. The incidences of the avascular necrosis is related with the degrees of the rotation into valgus(p=0.004).
CONCLUSION
The undisplaced abduction fracture of the neck of the femur is known to the result in good prognosis. According to this study, not a few avascular necrosis of the femur head happened. As a results, although undisplaced abduction fracture regained to be paid intention to the incidences of the avascular necrosis and careful follow-up should be accompanied.

Injury of Lateral Meniscus Associated with Tibia Plateau Fracture

PURPOSE
To analyse the frequency of soft tissue injuries associated with tibial plateau fracture through arthoscopy and demonstrate the pattern and treatment of lateral meniscal tear which is the most frequently concomitant injury. MATERIAL AND METHOD: We evaluated the charts, X-rays and arthoscopic records of the 27 patients who had been diagnosed as tibial plateau fracture and received the arthoscopic examination or arthoscopic assisted operative management and analysed the injury pattern, association of schatzker classification and treatment of the 9 patients who had been concomitant with lateral meniscal tear.
RESULT
In our cases, lateral meniscus tear was the most common among the soft tissue injury it was revealed that a longitudinal tear at the peripheral area was shown in 8 cases, which were treated with meniscal repair and a radial tear of the central area in l cases, treated with partial menisectomy.
CONCLUSION
Lateral meniscus injury is one of the most frequently concomitant with tibial plateau fracture, In those cases, arthroscopic meniscal repair would be helpful for better prognosis.

New Bone formation in Vascularized Periosteal Flap over the Bone Defect of the Shaft of the Radius in Rabbit. : Early Radiological, Histological and Immunohistochemical Study of Osteogenesis

Vascularized periosteal flap(VPF) it thought to enable formation of new bone and promote union in bone defect without risk of complications in donor site. Studiol about the VPF thus far have been centered on the long term result than early change after YPF. The purpose of this study was to elucidate the process of new bone formation in early stage after YPF by radiological, histological, electron microscopical and immunohistochemical studies. In experimental group, segmental resection of bone including the periosteum was performed in fifty-six radii of immature New Zealand white rabbits preserving the periosteal circulation from median artery which is the main source of blood supply to the periosteum. In order to simulate the transplantation of VPF, the vascular pedicle of median artery and veins was dissected from adjacent soft tissue and the periosteum was longitudinally incised to remove the bone tissue in the periosteum. Thereafter the vascularized periosteum was repaired. From the first to fourteenth day after the simulated VPF, the findings in the VPFs were observed by radiological, light microscopical, scanning electron microscopical methods and activity of osteocalcin was measured by immunohistochemical method. In control group, the bone tissue and periosteum were completely removed from the mid-shaft of seven radii, thereafter the radiological findings were observed at 1, 2, 3, 4, 8, 12, 16th week and light microscopical findings were observed at 8, 16th week after operation. From the results of this study, it is concluded that YPF is vigorously and uniformly osteogenic in its early stage. It it thought that this study could serve as a basic data for the future experimental studiol about VPF in animals and clinical application.

A Simplified Technique of Distal Locking Screw Fixation of Interlocking Nail

Failure to determine distal femoral screw hole position can sometimes prolong operating time for placing interlocking screw and increased radiation hazard. We attempted to assess progress in the insertion of distal locking screw with target device. Insertion method of distal locking screw in femoral nail that uses target device improved the accuracy of distal screw placement and reduced the radiation exposure. The authors analyzed 30 patients(33 cases) of the femur shaft fractures that treated by interlocking IM nailing using target divice in the Department of Orthpaedic Surgery, Chung-Ang university from August 1990 to July 1994. Among these patients, except 6 cases, all of the distal femoral drill holes attempted were successfully made with the first pass of a dirll without image intensiner monitoring. Compared with the commonly used free-hand method, our target divice assisted screw placement offer a reduction in the amount of time and radiation exposure to insert distal locking femoral screws.
