- PURPOSE
To analyze the clinical results of the treatment of Mason type II radial head fractures using closed reduction and K-wire internal fixation under C-arm guide by radiologically and functionally. MATERIALS AND METHODS Between March 2001 and October 2003, 7 patients with Mason type II radial head fracture were treated by closed reduction and internal fixation using K-wires under C-arm guide. The average age of the patients was 38 (5 to 57) years old, and average duration of follow up was 20 (5 to 36) months. At last follow up, we evaluated the radiological results and functional results by classifying excellent, good, fair and poor according to functional rating system of Broberg and Morrey. RESULTS The range of motion of the elbow at last follow up, average flexion contracture was 1.4 (0 to 10) degrees, further average flexion was 146.4 (140 to 150) degrees, average supination was 74.2 (70 to 80) degrees and average pronation was 75 (70 to 80) degrees. In the functional results, 6 cases were excellent and 1 case was good. In the radiological evaluations, the average time of union was 5 (4 to 6) weeks after the operation and no serious complication was occurred in any cases. CONCLUSION In the treatment of Mason type II radial head fracures, closed reduction and K-wire internal fixation under C-arm guide was an effective method of treatment with satisfactory results and no complications.

- PURPOSE
To investigate the usefulness of closing wedge osteotomy with threaded steinmann pin and wiring for the treatment of cubitus varus deformity after elbow fracture during childhood. MATERIALS AND METHODS From February 1994 to February 2002. We performed closing wedge osteotomy with threaded steinmann pin and wiring in 16 elbows with cubitus varus deformity. There are 11 men and 5 women. Mean age was 21.6 years and mean follow-up was 19.2 months. Mean deformed carrying angle was varus 21.7 degree. Mean period from initial injury to treatment was 16.5 years. RESULTS Mean angle that was corrected by above operation methods was valgus 12 degree. Average periods of immobilization was 27.8 days. One tardy ulnar nerve syndrome before surgery was solved at 8 weeks after operation. 2 cases with superficial infection was treated easily. 14 cases of all were estimated as good with Oppenheim's criteria. CONCLUSION Closing wedge osteotomy with threaded steinmann pin and wiring makes early range of motion exercise being possible as rigid fixation. The supracondylar closing wedge osteotomy with threaded Steinmann pin and wiring is thought to be the useful method.

- Vascularized fibular graft is one of the well accepted methods in the treatment of large bone defect of femoral shaft. But bone fixation with Ender nails through the same incision of bone graft has never been reported. We performed vascularized fibular graft and bone fixation with Ender nails through single medial skin incision and permitted physiologic stress. We achieved early radiologic union and medullary widening and the patient could return to work 9 months after the accident. We would like to report such an experience of treatment with the reference of literature.

- PURPOSE
In many cases of open comminuted supracondylar fracture, it is very hard to apply traditional methods. So we have used Ender nails for temporary transarticular fixation. We thought that it could provide enough stability to control the wound and didn't promote further soft tissue damage or infection. We performed 2nd stage rigid fixation and bone graft after soft tissue healing. We have studied to prove this staged operation valuable to treat the very severe open comminuted supracondylar fracture of femur. MATERIALS AND METHODS We analysed 16 cases which have been treated with such staged operation method from April 1992 to April 1996 about complication, union time and functional result in retrospective method. RESULTS We could prevent severe wound infection in all cases. We performed 2nd stage rigid fixation and bone graft average 6 weeks after first stage temporary fixation.
The average union time was 8 months and average range of motion was 10degrees flexion contracture and 100degrees further flexion. CONCLUSION In patients with very severe open comminuted supracondylar fracture of femur, the temporary fixation with transarticular Ender nails allowed the successful initial management for the secondary rigid fixation and bone graft and time could be saved for management of concomitant injuries. So this new staged operation is considered as a good method for safety, union time and functional result.
