Usefulness of CT Scan in Treatment of Calcaneal Fracture

PURPOSE
Accurate fracture classification is difficult because of its complex bony architecture and there is no definitive treatment modality according to fracture pattern or classification. We evaluated the fracture pattern using simple radiography and CT scan simultaneously and then the result according to treatment modalities and fracture classification.
MATERIALS AND METHODS
We evaluated 24 patients (26 cases) who suffered intraarticular calcaneal fracture were treated with closed reduction and axial pinning or open reduction and plate fixation. There were had taken CT scan. Fractures were classified with two different manner using both plain radiography and CT scan. Essex-Lopresti classification was made with plain radiography and Sanders classification was made with CT scan. Radiographic results were measured of preoperative and postoperative Bohler angle on plain x-ray. 16 patients (17 cases) were analyzed for clinical outcomes by Salama's criteria focused on pain, patients' satisfaction, walking abilities and usage of orthosis.
RESULTS
Mean followed-up period was 6 years (range 1~8.8 years). 8 cases were classified into tongue type intraarticular calcaneal fractures by plain radiography and 9 cases into joint depression type. According to Sanders classification by CT scan, 5 cases were classified into type IIA, 1 case IIC, 4 cases type IIIA, 4 cases IIIB and 3 case type IV. 12 fractures were reduced by closed reduction and axial fixation and 5 cases by open reduction and internal fixation. There was no correlation between clinical outcomes and treatment modality but there was linear correlation between clinical outcomes and postoperative Bohler angle (correlation coefficient = 0.04). In 8 cases of tongue type by plain radiography, sanders type IIC was only 1 case which calcaneal tuberosity connected to articular facet but articular involvements were observed in remaining cases.
CONCLUSION
Accurate eavaluation of articular facet in calcaneal fracture by CT scan is necessary to determining to select the treatment option.

Surgical Treatment of Internal Malleolar Fracture of the Ankle: Rush Rod Versus Plate Osteosynthesis

PURPOSE
We evaluated the results between the methods of open reduction and internal fixation using plate and screws and the methods of closed reduction and fixation with rush pin in lateral malleolar fractures.
MATERIALS AND METHODS
We analysed the 33 fractures of lateral malleolus which had been treated by open reduction and internal fixation using plate and screws or closed reduction and fixation with rush pin from January 1995 to January 2002 and had been observed over 1 year. The 33 patients were observed for the comparison of radiologic and clinical results in according to the measure of McLennan and Ungersma.
RESULTS
Among the 33 cases, 15 cases were treated by open reduction and internal fixation with plate, and 18 cases were treated by closed reduction and Rush rods fixation. In according to the measure of McLennan and Ungersma, good radiologic result was 60% (9 cases) and excellent clinical result was 27% (4 cases) in plate fixation, and good radiologic result was 61% (11 in 18 cases) and excellent clinical result was 39% (7 in 18 cases) in Rush rods fixation.
CONCLUSION
In ankle fractures of elderly patients who have soft tissue problems and osteoporotic bony quality, radiologic and clinical results of internal fixation of distal fibula were relatively same between fixation with plate and screws and Rush rods. Therefore, closed reduction and internal fixation with Rush rods is one of the good treatment modalities of distal fibular fracture.

The subtrochanteric fracture of the femur is often comminuted because the bone here is mainly cortical and high velocity trauma and hiornechanically and adjacent power muscles. Therefore, it is difficult to maintain accurate reduction and rigid fixation and delayed or nonunion, limb shortening, varus deformity, mechanical failure, and many other problems can be encountered. We studied 6 comminuted subtrochanteric fractures treated with Ender nails from Sept. 1991 to Oct. 1992. 3 weeks of skin or skeletal traction followed by partial weight bearing postoperatively showed excellent outconles.

In cases of unstable thoracolumbar burst fractures, recently the operative treatments such as The population of the old aged person in Korea has been increasing rapidly since 1960s, The proportion of over 65-aged person was 2.9% of total population in 1960s, but which has been increased rapidly to 5% in 1990 and is expected to 6.8% in 2000s. Therefore, according to the increasing number of osteoporosis, trochanteric fractures in old-aged group will be expected to increase rapidly. So we should give more attention to the further comprehensive treatment and classification of trochanteric fractures.

The stress fracture is a disease which results from the application of an abnormal stress to the normal bone by the action of the constant and repeated muscular pull. prior to the early 1960s, most reports of stress fractures were from military installation, however, with the recent increase in participation in leisure and professional athetic activities such fractures have vecome more common among civilians. We treated two cases of stress fractures of the supracondylar region of the femur in runners. One patient was treated conservatively, but the other with displacement was treated operatively. To our knowledge, no previous cases of this nature have been reported.

Dislocations of the distal radioulnar joint without fracture are more common than would be expected from the literature and most of these injuries are not diagnosed when seen initially. Several chronic problems may befall the distal radioulnar joint-loss of forearm rotation, chronic pain and arthritis, and a great many surgical procedures have been devised to relieve them. Six patients were treated with resection of ulnar head (Darrach Operation)in 3 cases, ligamentous stabilization(Hui and Linscheid Operation)in 3 cases, and we found more satisfactory results in the latter.

To determine whether any of the commomly used wiring technique are rigid enough to allow early motion in treatment of patellar fracture. Twelve cases of extensively commivuted and displaced patellar fracture were treated using wire technique (circumferential combined tension band wiring) at N.P.H. from April 1987 to April 1989. This method has advatages of simple and rigid fixation and results were satisfactory.

One of the greatest diagnostic challenges that faces both orthopedic surgeons and the radiologists is the patient with a subacute or chronic wrist injury who has no obvious clinical or radiographic abnormality to explain the pain. The wrist arthrography is used to evalute structures that can not be seen on plain radiography. These structures include the synovium, the intraarticular ligaments and the articular cartilage including the triangular fibrocartilage. The most inportant indication is persistent pain or limitation of motion after trauma. We think that the wrist arthrography is to be used widly. We collected and analized the results of wrist arthrographies performed in 33 patients with traumaic painful wrist.
